Categories of Sleep-Disordered Breathing

  • Home Sleep Apnea Testing (HSAT): Measures respiratory effort, airflow, and blood oxygen saturation. Highly efficient for detecting moderate-to-severe OSA. However, lacking EEG (brain wave) capabilities, it cannot detect micro-arousals caused by respiratory effort without accompanying oxygen drops.

  • In-Lab Polysomnography (PSG): The definitive diagnostic gold standard. PSG captures comprehensive neurological, respiratory, and cardiac data, making it clinically necessary for diagnosing central apneas, complex apneas, and ambiguous hypoventilation states.

Therapeutic Interventions & Management

Positive Airway Pressure (PAP) Modalities

  • CPAP & APAP (For OSA): Continuous Positive Airway Pressure (CPAP) acts as a mechanical splint, utilizing pressurized room air to keep the airway open. Auto-Adjusting PAP (APAP) uses algorithmic flow-limitation detection to dynamically adjust pressure.   • OTC "Boil-and-Bite" Mouthguards: Non-titratable and exert uncalibrated pressure, leading to severe Temporomandibular Joint (TMJ) disorders and permanent bite misalignment.

  • Nasal Strips: Biomechanically incapable of preventing a pharyngeal (throat) collapse.
